The Division of Health Care Access and Improvement (HCAI) was created to promote the availability of quality health care services! in NYC through various programs. This is being accomplished by creating mechanisms and tools to promote access to quality health care for the uninsured, underinsured, and for inmates housed in New York City correctional system; oversight and evaluation of health insurance programs and health services to ensure high quality and maximum access for targeted populations, including those with special needs; promoting a general awareness and understanding of available health services; and providing leadership to health care and human services community in planning and developing programs that support the mission of the Division. HCAI is charged with overseeing and improving systems of health care for people in New York City and has a staff of approximately 500 employees and an operating budget of $222 million.

Within the Division of Health Care Access and Improvement, the Bureau of Correctional Health Services-Transitional Health Care Coordination (THCC) has a vacancy f! or a Special Consultant (MHSS) Level II to function as the Dir! ector of Transitional Services Projects. The candidate selected for this position will be responsible for providing consultation and technical assistance services to grant funded contracted programs. He/she will evaluate and monitor community based agencies providing transitional services including case management, mental health and substance use services to those transitioning from jails to the community, by contract or grant agreement, under the review of THCC, including Ryan White services to assure compliance with the provisions of applicable laws, regulations and agreements. The Director of Transitional Services Projects assists in planning and developing programs to facilitate implementation of case management, housing assistance, and mental health and substance use services to facilitate continuity of care and treatment from New York City jails to the community. This includes supervision of project analysts and correctional counselors in the performance of their duti! es. THCC services are integrated with other programs in the Division including Correctional Health Services (CHS) and Primary Care Access and Planning (PCAP). It also has partnerships with other DOHMH programs, the Department of Correction, District Attorney’s Offices, Public Defenders, other City agencies, and community-based providers throughout the City. In addition, the Director will work closely with the jail based health providers to facilitate continuity of care from intake jails to community treatment for those in City jails including those living with HIV/AIDs, with two or more chronic conditions, or one chronic condition and one behavioral health condition and soon to be released from incarceration and returning to the communities of greatest health disparities who face multiple social service issues.

Responsibilities

  • Responsible for administrative aspects of the program’s operations designed to facilitate court-ordered treatment ser! vices and placement in special programs to transition people soon to be! released from City jails to the community who need case management.
  • Make site visits to correctional facilities, courts and community programs to provide technical assistance in program development among grant funded jail, court and community-based programs.
  • Inform and implement administrative policies, procedures and regulations for uniformity of interpretation and practice and coordination of these policies and procedures with other areas in Correctional Health Services, the Division and the Department.
  • Coordinate collaborative meetings where justices, District Attorneys, public defenders, treatment providers and other community providers address various issues such as alternatives to incarceration, retention rates, continuum of care models; and develop subcommittees to address criminal justice initiatives, trends in care, coordination of services, criminal justice awareness campaigns, etc.
Minimum Qual Requirements

A baccal! aureate degree from an accredited college or university, and either:

1. A Master's degree in Social Work from an accredited college or university in the school of social work, or 60 graduate semester credits from an accredited university in social work, psychology, health, rehabilitation, public health, psychiatric nursing or special education; plus one year of full-time, satisfactory experience providing direct care in a social, psychiatric, health, mental retardation/developmental disabilities or substance abuse/chemical dependency/alcoholism agency or in the administration of a

program providing direct care as described above, or as a consultant on program planning or evaluation for such services; or

2. A Master's degree of at least 30 graduate semester credits from an accredited university in psychology, sociology, anthropology, public health, special education,

psychiatric nursing, counseling, human services, health, rehabilitation, ! public administration, or business administration; plus two years of ex! perience as described in "1" above; or

3. A satisfactory combination of education and/or experience. Candidates without either a Master's degree as described in "1" or "2" above or at least 60 graduate semester credits in the areas listed in "1" above, must have at least:

a) a baccalaureate degree from an accredited college or university and one year of actual experience as described in "1" above; plus

b) a satisfactory combination of: (i) graduate semester credits in the areas listed in "1" and "2" above; and/or (ii) additional experience as described in "1" above, to equal 60 graduate semester credits or 2 years of experience. Graduate semester credits from an accredited university may be substituted for the experience in qualification "3 b)" only, on the basis of 30 graduate semester credits for each year of experience. Experience may be substituted for graduate semester credits in qualification "3 b)" only, on the basis of one year of e! xperience for each 30 graduate semester credits.

NOTE: For assignment to Assignment Level II, in addition to meeting the above qualification requirements, all candidates must have one additional year of experience as described in "1" above.

Preferred Skills

  • Master’s degree in social work or related profession
  • At least five years of experience in the field of criminal justice or public health
  • Strong experience working with courts, District Attorneys or Public Defenders
  • Familiarity with health and mental health service organizations
  • Comfortable working in a correctional setting
