As a member of the Hospice and Palliative team this position will; respond to requested consults, encourage open communication about end-of-life issues among all patients, families and health care providers, provide comprehensive pain a! nd symptom management assessments, collaborate with physicians, educate staff through in services and lectures, and the achievement of the best quality of life for patients and their family members.

Job Requirements
Master's degree in Nursing and graduate of an approved Nurse Practitioner Program required. Hospice and Palliative Care preferred. Minimum 2 years experience in end of life care. Hospice and/or Palliative Care experience preferred. Computer literate with knowledge of Microsoft word, excel and e-mail. Ability to operate medical equipment. Current NYS Registered Nurse License ANA/ANCC Certification ACLS/BCLS Certification. Advanced Practi! ce certification in Hospice & Palliative Care within one year ! of hire. Autonomy and independent thinking and decision making on a daily basis. Ability to work non-judge mentally with patient/family of any culture, religion, socio-economic background or life style. Strong, effective leadership skills. Effective collaborative and team building skills. Excellent communication skills both in writing and verbally. Proficient documentation skills. Good organization skills. Analytical, critical thinking, managing, coordinating and prioritizing skills. Ability to secure confidential matters. Excellent interpersonal skills.
